Rules:

Each purchased Primary Care Card is valid for one year.

The complimentary consultations offered through the Primary Care Card are accessible with specific Premium Medical primary care specialists. Please reach out to the clinic administration to coordinate the most convenient appointment times for you. In the event the client cannot make the appointment, the clinic should be notified at least 6 hours before the scheduled appointment time. If you miss an appointment without prior notice, the primary care session will be marked as used.
